In the JAGES Aomori Project, stakeholders collaborate todevelop and evaluate a sustainable model that realizes residents’ well-being,even under the severe conditions of rapid population aging and decline.
To achieve this, the project promotes primordial preventionand health promotion through environmental improvements; advances integratedimplementation of health services and long-term care prevention for olderadults; supports evidence-based policy making by local governments; fosterscollective impact among stakeholders within and outside the prefecture; anddevelops and mobilizes local resources, including human resource development.
